“Conspiracy of Silence” Short Film 1 of 50 in the LIFE Before Death documentary series about the global crisis in untreated pain and the dramatic life changing affect palliative care services can deliver to patients and their families around the world. Conspiracy of Silence exposes the needless implementation gap responsible for the suffering of millions of people around the world today. Lack of political will and general awareness of this pressing humanitarian cause are blamed for the appalling situation where 80% of all people lack adequate access to essential pain medicines like morphine. Pain and palliative care activists call for urgent change to this very treatable problem and challenge us to speak up for those less fortunate who have no voice and whose suffering is mute. "It goes down to the molecular. So we tend therefore to concentrate often on 'the liver', or what's happening in 'the brain'. And we forget that people are more than that. Even if you put all of those parts together you still don't get a person - we're greater than the sum of our parts. The pain area recognizes that. So it's a fascinating area because with a basic approach, and a compassionate approach, we're able to really make a difference in people's lives." | 3 | VideoLIFE Before Death Pediatric Palliative Care | DVD OUT NOW www.lifebeforedeath.com In this short film we discover that the barriers to pediatric palliative care are very complex and pain in children is often under treated. We learn that providing good care is crucial to the quality of life for the child but that support needs to extend to the whole family. "I think that in the pediatric population pain is more frequently under recognized and under treated than it is in the adult population, though the medications are available, sometimes the training for physicians is a little bit inadequate and physician's feel unsure in their ability and concerned about the medications," reflects Dr Kim Bower (USA). Dr Bower goes on to explain the importance of community involvement in a child's palliative care - especially when it comes to the parents and siblings -- but also the family's church or spiritual community as well as the school the children attend. Dr Bower explains that for kids, their desires and what they look forward to and want to do may be different to that of adults in that children want to enjoy time with their family and do normal and fun things like go to amusement parks. "It's important that families are supported to make this possible." | 4 | VideoLIFE Before Death Nurse in the House | DVD OUT NOW www.lifebeforedeath.com In this Short Film we learn about the innovative health policies in Uganda allowing nurses to prescribe oral morphine in the homes of patients. "Sometimes we visit patients in their homes because when the patient has to move it makes the pain worse," explains Martha Rabwoni (Uganda). "The pain if is not treated, the patient is in total chaos -- they cannot think, they cannot do anything, they concentrate all their mind on the pain -- they cannot eat, they cannot dress, they cannot bathe, they cannot move. The relatives in that home -- nobody sleeps." "Many developing countries still have a real problem about the use, importation and manufacture of morphine," explains Eugene Murray (Ireland). "The second thing is having appropriate ways to distribute it. In Uganda, they dilute powdered morphine into water which is which is colored with a dye to indicate the three different strengths and is distributed by nurses in a community using recycled water bottles. That may seem very crude but in terms of pain control that is absolutely transformational." Opiate medications, which are medicines derived from opium, are considered by the World Health Organization to be the gold standard in the treatment of moderate to severe pain. Criminal intent to divert important legal medicines by black market drug traffickers is identified as one of the contributing factors to opiophobia. This fear is reiterated by the testimony of late stage breast cancer patient Carmen (Australia), who at first resisted the opiate medicines that ultimately treated her pain effectively and gave her back her quality of life.
