Hello I'm Libby Stewart, I'm Aesthetic Nurse Practitioner here at Aurora Clinics. Well, after you peel what do you do next? How will it feel and how can you optimise your treatments? Following your procedure after the sunscreen has been applied to your skin you go home and do nothing, just go to bed, don’t wash your face, nothing else for the rest of the day. The following morning you can start again with your regular skincare routine. That day your face may feel a little bit tight and may appear just slightly red, a bit like as if you’d been out in the sun the day before. That is perfectly normal. The following days, days 2 and 3 you might start to see some shedding of skin cells, some very slight flaking. This is normal too, as is actually not to see any flaking at all. The way the peel works is to loosen the skin cells bond to the skin and sometimes this is very minor in the early days after applying a light peel and you may not actually see anything, but this does not mean that your skin peel has not worked, you will be achieving the results you required. The very best results will be obtained by having a course of peels which will be spread over a time of three months, one peel every month for three months give optimum results. The very finest results will be achieved by the use of our specialised products made by the Rationale Range which compliment our peels. These are only available through physicians clinics on the prescription of your surgeon, doctor or nurse practitioner, and these are available via our online shop or here at the clinic. Hello my name is Libby Stewart and I work here at Aurora Clinics as one of the team of Aesthetic Nurse Practitioners. How would you prepare your skin for a skin peel and what could you expect during treatment? Following your consultation with your practitioner you will be advised on a skincare regime which you need to complete for two weeks prior to partaking in your peel. Your peel appointment will be for about half an hour, although the actual procedure only takes about 15 minutes to perform. You will be lead on a bed and some gauze placed over your eyes just to protect them through the procedure. Your face will be cleansed with a very light non-foaming cleanser and then a layer of the chosen prescription product will be applied to your face. You may or may not feel anything during the procedure. Sometimes the face feels rather warm and sometimes it can feel like a very light stinging sensation equivalent to a sort of nettle type sting. This is self-limiting and disappears within about a minute. Depending on the issues that you wish to address, the fine lines or congested pores, maybe another layer will be applied, maybe sometimes just to specific problem areas, or sometimes just to target the areas that you wish to address, and maybe made of a different type of chemical. This will have been decided in your pre-consultation. As soon as your skin has settled, which does take just about a minute, a layer of moisturising sunscreen will be applied. You will be able to get up then and go about your business for the rest of the day. Your face will look a little bit flushed like as if you’d just been at the gym, but that fades very quickly and the sunscreen we put on comes in a choice of natural or a slightly tinted, so you can go back to work or pick up the children from the school run, whatever it is that you need to do for the rest of the day. As I said, the procedure is very, very quick, it only takes about 15 minutes, so the appointment is half an hour and we will be very happy to see you here at Aurora if you’d like to call us or contact us via email. Hello my name is Libby Stewart and I work here at Aurora Clinics as one of the team of nurse practitioners. What are the advantages of different types of peel? Well when you consult here at the clinic your practitioner will help you to decide on the right prescription based on your skin type and the specific problem area that you wish to address. The areas that you may be looking at would take into account maybe fine lines, pigmentation changes brought about by sun damage, blackheads, rosacea, the open pores or just a general unevenness of skin texture. After the right prescription type has been assessed for you we would start your treatment conservatively by applying a light layer of chemical to your face. This would be reviewed after a month and depending on the results of that treatment we might change the chemical formulas in the peels, or maybe increase the level at which we’re working. For people with more severe problems or a skin which has aged considerably who are considering a face lift, a deeper peel may be performed at the time of their procedure in theatre. These procedures are often shown on television programmes such as 10 Years Younger and do bring with them a longer down time or recovery time than the light to moderate peels which are performed here at the clinic. Hello my name is Libby Stewart, I'm one of the Nurse Aesthetic Practitioners here at Aurora Clinic. Who is suitable for a chemical peel? Well, apart from ladies expecting babies, most people of any sex over the age of 16 would be suitable for a peel of some sort. The suitability of your skin type is assessed here at clinic with your practitioner using something called the Fitzpatrick Scale. The scale indicates to us the level of exposure that your skin has experienced due to sun and other environmental factors, and we do also look at the aging of your skin as part of that consultation. But anybody over the age of 16, irrespective of the colour of their skin or indeed their skin type, will benefit from some peel or another. As peels are customised for your specific needs there is no need to worry that we wont have something to help you. Peels will help with sun damaged skins and also skins that have areas of uneven pigmentation caused through sun damage or perhaps through skin conditions called rosacea, or other conditions of that type. Peels will help hydrate the skin and will lighten and refresh the skin complexion giving it a natural healthy glow. We can help acne, as long as it's not in the active stage, with peels as well. Blackheads, open pores and uneven texture are all able to be assisted with the use of chemical peels. Hello my name is Libby Stewart, I'm an Aesthetic Nurse Practitioner here at Aurora Clinics. Sometimes people come to the clinic to talk to us about their skin and what can be done to help with specific problems that they have with their skin. They may have heard of chemical peels but are unsure that this is a treatment that is suitable for them, or indeed may sometimes be a little scared by things that they have seen and read about chemical peels. What is a chemical peel, and exactly what does it do for you? A chemical peel is a chemical treatment which is applied in either a theatre procedure or in an outpatients procedure for lighter to moderate peel. The heavier peels being done at the theatre procedure. Its aim is to refresh the skin and stimulate cell turnover. It can help treat specific needs of the acne, sun damage and general hydration, lightening and brightening. Here at the clinic we would customise each peel to a patient’s specific needs following a consultation with your practitioner. The light to moderate peels are done here at clinic, they have a very short recovery time, and the heavier peels will be done in theatre often as part of a procedure as sometimes seen on television programmes such as 10 Years Younger. Aurora’s nurse specialist Libby Stewart explains what to expect during treatment and how you will feel immediately afterwards. Hello my name’s Libby I'm Aesthetic Nurse Practitioner here at Aurora Clinics and we’re talking a bit about dermal fillers. In this little section of the video I wanted to explain what happens before treatment and what to expect during treatment. Your first consultation with the clinic we’ll take a complete medical history from you and like I said in the first video most people are suitable for treatment. We will also take some photographs of you so that we have a baseline to gauge the result of your treatment which is immediate. There is no specific preparation that you need do before you come to clinic for the treatment. The treatment time is a very short treatment, it takes about 15 to 20 minutes to perform the procedure although your appointment time is obviously longer as we put some anaesthetic cream on your face or other areas that we can treat which are perhaps backs of the hands to improve aging, and the décolletage area, but local aesthetic cream is applied and we wait for about 20 minutes for that to work. The procedure itself, you will be sitting on a bed and we’ll clean the face with a product such as Witch Hazel, something to clean the skin and make sure there is no bacteria so to minimise infection during the procedure. The procedure itself is performed by the introduction of a very fine needle underneath the skin and a very slow injection of a colourless smooth homogenised gel which does lift a line so that we have immediate results. Following the application of the gel the product is sometimes massaged into place, not always, it just depends on the depth of the line in the product that we’re using, but sometimes a little massage is required and following that we would sooth your skin by applying a cream such as Oraderm which is a vitamin K cream which helps prevents any bruising which might be caused. Bruising is very minimal but we are introducing a product into the skin with a fine needle so a little redness and bruising can be expected in some people. Really that is the extent of the treatment, immediate, fast and long-lasting. It will last about anything between six months and twelve months depending on the area we place it. A little maybe shorter lifespan if we inject around the mouth or areas of very sort of high movement or vasculature, but a very good treatment and with immediate long-lasting results. Aurora’s nurse specialist Libby Stewart explains that they are often used on the face to soften mouth to nose lines and marionette lines which come downwards away from the corner of the mouth. Hello my name’s Libby Stewart I'm Aesthetic Nurse Practitioner here at Aurora Clinics. So who is suitable for treatment with dermal filler and where would you expect dermal fillers to be placed? Well the who is quite simple. Most people are suitable for treatment with a dermal filler. Obviously treatments have never been approved in ladies who are expecting babies so we have no data to support that and wouldn’t treat anyone who was pregnant. There are certain auto-immune diseases and people with recurrent sore throats etc that we would keep an eye on, but these are not particularly contra-indicated and we would discuss this when taking your medical history during your consultation. The treatments are perfectly safe as we are using a natural product which is biodegradable and would only expect to last in the body for about nine to twelve months. It is something which the body is able to break down quite naturally, and whilst perhaps this might not seem great if you have to keep getting your treatment done every year, it does mean that you get a nice fresh result and ideas of beauty changes, and your face changes around it, the hyaluronic acid filler only stays in place whilst it is suitable for that particular area. Most people who present at clinic are concerned about the lines that come between the nose and the mouth here which are called the nasal labial lines. These are the ones which really sort of show the sign of ageing first as we lose our fat pads here underneath our cheeks and that skin has nowhere to go but downwards and can cause that very deep crevasse there and those fill out very nicely with hyaluronic acid fillers and really produce a very startlingly good result which is immediate to see. Other places that we can use the hyaluronic acid in the face are the areas here which are called the oral commissures where we have the volume loss here that again is created by the aging face, and this can be padded out to the natural volume that was apparent in the more youthful face. Lines which ladies really hate are the ones going between the corners of the lips here down towards the jaw which are called the marionette lines, which give the appearance of a sort of puppet-like face, and with a little more volume in here and the relieving of the creases here really do help restore that balance. Fillers can be used to restore volume in places like the cheeks where perhaps you want more volume than you used too have in that part, so we can use it for creating a sculptured look as well as just re-creating what you had before. Aurora’s nurse specialist Libby Stewart describes how the gels are made and discussed the benefits of individual brands. Hello I'm Libby Stewart one of the Nurse Practitioners here at Aurora Clinics. People have heard of dermal fillers but they’re not quite sure what they are perhaps. So what are dermal fillers and what choices do you have when choosing a dermal filler? A dermal filler is an injectable treatment of a gel which is placed underneath a static line or wrinkle to either improve the appearance of the wrinkle or perhaps to replace volume that has been lost in the aging face. There are many types of fillers on the market, but here at Aurora we prefer to use the fillers from the Allergan range, you may have seen some advertisements in the press recently for the Juvéderm range which is the latest in the range, or the very dependable Hydrafil range that was the forerunner to Juvéderm. These fillers are made from something called hyaluronic acid. They’re a natural based substance which is common to all tissue types. Trees, plants, animals, humans all have hyaluronic acid and it is this hyaluronic acid that is responsible for the hydration and soft moving of our skin, our eyes, our joints, so it is something that is essential to us but its production does decrease as we age. Its natural life is very short-lived in the body so even if we were to inject pure hyaluronic acid it wouldn’t last for very long, so it has to be stabilised in a gel, and the basic choice between products comes between the carrier gels that hold the hyaluronic acid. The Allergan range is a smooth homogenous gel which goes underneath. Other companies have gels which are based on particular products which hold their shape and give their longevity, but the Allergan range has formulated a matrix approach which helps to maintain the volume underneath the line once it has been corrected. So that is the main choices between whether we would use one brand or another, but that would be assessed according to your needs and your desired outcome at your consultation with your practitioner. So my basis is a surgeon, I'm very interested in non-surgical side of medical care, particularly Botox, dermal fillers. We’ve done a number of studies on this, and I'm also very involved in the Premier teaching course in the United Kingdom, teaching other practitioners how to use these products and that’s cosmetic courses which I've been involved in for the last six years. So what we aim to do is really offer you a very high quality personal service. I work very closely with a set of nurses, aesthetic nurses who have all been very highly trained and work under my supervision. We’ve also got other doctors who work closely with us.
