iDown works by monitoring information from the accelerometer embedded in your iPhone.

If you fall, iDown employs the Smart Fall™ detection formula triggering an instant e-mail or text message to your designated recipients, providing your GPS location and street address. In addition, you have the option of instantly activating a touch switch that will provide the same alert information to your recipients, even if you have not fallen.
