CleanSarcopenia | Sarcopenia leads to uncomfortable and often irreversable health problems Sarcopenia is a medical term that means the muscles in your body are deteriorating or shrinking. This condition is more noticeable in the baby boomer population and older. When we hear about older people getting smaller, it’s due to this condition. When your muscles shrink it causes you to become weak and fragile. This puts pressure on your bones. Sarcopenia leads to many other conditions that are common in old age. It starts with having no muscles to becoming imbalanced and falling and breaking bones and going to the hospital. Then people often end up in nursing homes because they can’t fully recover. This can lead to pneumonia, or getting a blood clot because they are not moving around, and then they can’t get out of bed. This is downward spiral that all starts with muscle loss. Loss of testosterone is what causes sarcopenia. Unfortunately people who are well into their 70’s and 80’s can’t be cured of sarcopenia. The problem can be addressed for it to not get worse but it can’t be reversed. It can be prevented by the replacement of testosterone. People who undergo HRT are less likely to get sarcopenia because their muscles will stay strong and healthy. They won’t become weak and frail; they won’t deal with curvature of the spine. CleanProgesterone and PMS | The lack of progesterone leads to PMS The hormone progesterone has created many theories surrounding PMS. When women are stressed they stop making progesterone and then they are no longer fertile. Progesterone gives mental stability. A woman at any age can have PMS unless they are premenstrual or menopausal. Men don’t ever have progesterone and there is no reason for it to be given to them. Sometimes progesterone can help women in place of anti depressants or anti anxiety medications. The methodologies of delivery have evolved over the years. Taking bioidenitical progesterone orally turns it to estrogen in the liver and then you are made worse not better. Synthesized estrogens also don’t help. Vaginal suppositories then ready tabs that dissolve in the v****a, or tabs under the tongue go directly to your blood stream ad not to your liver. Vaginal tabs and sublingual tabs can be used once a day. Creams are fine but they have to be applied 6 times a day. Progesterone causes drowsiness so you want to take it once at night. Pellets are the optimal choice for menopausal women because you only have to think about them once every 4 months. During menopause if a woman has a uterus progesterone is given as well so that the lining of the uterus stays stable. Giving too much progesterone causes the lining to thicken too much and this may lead to cancer. Progesterone helps to balance the uterine lining. It’s necessary to keep you from getting uterine cancer. CleanThe Cascade of Aging | Progesterone is the second hormone to fall, testosterone is the first. Hormones protect and defend women against the aging cascade. Testosterone is the first line of defense. The second defense is progesterone. Testosterone slowly decreases between ages 38-50. Progesterone is an intermittent hormone. It is secreted from when we start having periods to when we stop in the second half of the cycle. Progesterone is a hormone that prepares the body for pregnancy. When progesterone is low and estrogen is too high, it makes body unable to carry a pregnancy and causes irritability. When our bodies are healthy, our progesterone levels are normal so that we can carry a baby. When we are unhealthy, either too much exercise, unhealthy diet, etc, our progesterone levels are abnormal and therefore make it difficult to carry a baby. When we’re young we have lots of progesterone and it keeps us calm. When we have less of it causes what people call PMS. PMS is actually the loss of balance in progesterone levels. Medicine still claims that PMS is due to psychological conditions and not to progesterone. However, the psychological issues exist because of the lack of progesterone. Unfortunately, although most OB’s understand the causes around PMS, the leaders in American Medicine do not recognize it properly. In hunter-gather societies, when there was a lack of food, women’s progesterone levels droped so that they were no able to make babies that will come in and demand the tribes’ resources. Once food sources increase, progesterone returns and many babies are born in the same time period. CleanCurrent topics in Anti-aging Medicine, Part 2 | Qualified Anti-aging Doctors and Safe Compounding Pharmacies Episode 62 of the BioBalance Healthcast is a continuation of a discussion started in the previous podcast based on an article published on the CNN Health website on Wednesday December 28, 2011 titled The risks of anti-aging medicine. The article alleges that anti-aging doctors are not as well trained as other specialist. Dr. Maupin disputes this saying many anti-aging doctors are very well trained and qualified to do what they do. Anti-aging medicine includes medical advancements that make it possible for us to live longer lives, allowing us to enjoy better health and mobility in our later years, instead of being stuck in a nursing home for 20-30 years. In regards to Dr. Thomas Pearl’s comment on anti aging doctors, it’s important to understand that this physician is treating geriatric patients. Dr. Maupin does not suggest that hormone treatment therapy be given necessarily to this demographic. At that point patients may be too old for the therapy to make a difference. But patients that are menopausal or pre-menopausal can be helped by HRT. By offering this therapy doctors can prevent many diseases that are secondary to the loss of hormones. Diseases can be offset for 10-20 years with HRT. HRT is a treatment for a syndrome, not symptoms, and often save money and time. Chelation therapy for heavy metal toxicity is not done by BioBalance Health. Chelation has risks of renal and liver problems. There are alternative supplements to rid patient of heavy metals. Baby boomers have higher chance of heavy metal toxicity due to lead, mercury, and other harmful substances in food and the environment. Compounding pharmacies used to be popular 70 years ago before drugs were made by pharmaceutical companies. Current compounding pharmacists have the ability to compound drugs prescribed by a physician. Dr. Maupin supervises the use of compounding pharmacies for her patients. Her estradiol and testosterone pellets are made at a compounding pharmacy that is reputable and trusted. If your physician writes a prescription for a drug and sends you to a compounding pharmacy, be certain he/she has a relationship with the pharmacy they use. The FDA does oversee compounding pharmacies. The article sites information based on the Women’s Health Initiative study which since released has been discredited. Dr. Goldstein is using a bad argument, a bad study, bad information and miss-representing the use of bioidentical hormone therapy. Finally, Dr. Maupin discusses the term “the standard of care.” This term means the least amount of medical treatment that is acceptable by a physician. CleanCaring for Dry Skin | Winter weather leads to dry skin. Dr. Maupin offers advice for taking care of face and body. Episode 60 of the BioBalance Health features a look at how to treat dry skin. Dr. Maupin created a her own botanical skin care products that includes a line specifically for dry skin. The are available for purchase at biobalanceskin.com. Dry air can effect anyone’s skin. For men and women, it is important to have a cleansing routine for your face and body. A nightly routine is not just about removing makeup. It’s about keeping your skin healthy. Start your routine by washing with a cream-based wash. Using an alcohol based wash during the cold months will speed up the drying of skin. After you’ve washed your face, apply a toner to open pores and clean off all dead skin. Wiping with a toner will remove what your wash left behind and prepare your face for moisturizer. Moisturizer is meant to put a mask over the skin so all of the moisture produced in your face is held in while you sleep. Dehydration leads to dry skin regardless of the amount of moisturizer used. For your eye’s, you first need to use an eye makeup remover. BioBalance Health has one with the same PH as tears and is therefore hypoallergenic. You then need to use a product that contains vitamin K to lighten dark circles and arnica to stop puffiness. The gel dries but does not dehydrate under the eyes. CleanDepression — Diagnosis and Treatment | Depression has Physiological and Emotional Components Episode 56 of the BioBalance Healthcast is part one of a two-part series on depression. Next week's podcast will cover depression resulting from hormone imbalnce, but this week the focus is on a variety of topics related to diagnosis and treatment of depression, covering it from two perspectives; the medical and the psycological. In Dr. Maupin's medical practice and Brett Newcomb's counseling practice, patients often complain of or exhibit symptoms of depression. There is an overlap between their two fields. Brett often sends his counseling patients to a medial doctor to be a evaluated for health related causes. There are two types of depression: endogenous, that is having physical system malfunction, has to do with neurotransmitters in the brain. Exogenous depression is caused externally, usually by things that happen to people. We all have mood swings. If you have clinical depression, your mood goes way below the normal bottom point. Clinically depression is when you don't cycle normally, but instead stay at a low level. Antidepressants are often given. Proper evaluation is done in multiple hour long sessions. Exogenous depression can lead to endogenous change. Some people have a genetic weakness, e.g. have relatives on medication for depression, are hospitalized, or are alcoholic. Doctor patient relationships: doctor feels the patient's pain when the session is over, and the patient feels better, like they transferred it to the doctor. Therapists are are trained to protect themselves from becomming depleted from depressed patients who come in to the session get energy from the therapist. The have to llearn to give energy but still have some left after the session. Some people siuffering depression need medicine. Depression can be caused from a lack of hormones or long term stress. When cortisol is high for a long time, it wears out the neurotransmitters. When the patent is trying so survive a long term stressful condition, they can't get out of it without medication. Next week they will talk about esthetic laser treatments. Abbie Calabio is a board-certified Nurse Practitioner and esthetic skin specialist, only one of three in the St. Louis area. She has five years of expertise in the field and her specialities are cosmetic injections and laser skin treatments. In her work as a esthetic nurse practitioner, she combines science and artistry, sculpting the patient's face with injections of purified proteins. Botox® blocks the signal that causes muscle contractions that cause wrinkles. It is injected around the eyes to give a refreshed look, brightening the eyes. It is injected between the eyebrows where to remove what is referred to as elevens and outside the eyes as crows feet. All treatment begin with a consultation where she ask the patient what bothers them about their skin. She then addresses these concerns with the best medical solution. Radiesse® and Juviderm® are dermal or facial fillers. Radiesse® is a volumizing filler, a calcium based filler that helps enhance the volume, bringing fullness back to the patient's cheeks. CleanOff-label Use of Medicines as it Relates to Testosterone | Bioidentical Testosterone is safe and legal as a treatment for symptoms of menopause. Common Off Label Drug Use Use of approved drugs to treat diseases for which they have not been approved at the time it passed the FDA testing is so common there is even a name for it: “off label” use. There are many examples. Terbutaline was the generic name for an asthma drug produced between the 1970s and 1990s. During this time, although it was never approved for any purpose other than asthma, it was the primary subcutaneous and IV drug used to stop premature labor. Ramifications Off Label Sometimes ramifications of off-label use are significant beyond the simple expansion of drug use. Terbutaline, for instance, has been replaced in the last decade with a similar and more expensive drug for its non-approved use, prevention of pre-term labor. As a result, it was never approved by the FDA for this purpose; until it was replaced, however, it saved the lives of millions of babies. Why Not Approve Off Label Usage? If a medication has become generic, then it is not patentable, or “own-able” exclusively by any company. As a result, there is no opportunity for profit. It is therefore no longer cost-effective for a pharmaceutical company to pay for this high cost of research on the second use of the drug when the company has no possibility of realizing a return on the investment, much less a profit. And if there is no pharmaceutical company championing a drug for a certain disease, it is unlikely the FDA will approve it for that that purpose, regardless of its proven effectiveness! Why Not Approve Hormones? Hormones like testosterone fall into a similar category as generic drugs. Because hormones are natural, they are not “unique, and hence not patentable. With no financial incentive to produce unpatentable drugs, there are no investment dollars to support testing in a way the FDA requires. So generic drugs and natural hormones like testosterone remain untested for any but a narrow set of uses. Misuse of Hormones The first issue centers around the age and medical condition of the patient using it. Most negative reports are related to the use of testosterone in young, healthy men – usually athletes and body builders -- who naturally have excellent levels of testosterone, and who are attempting to artificially augment their testosterone beyond normal physiologic levels to increase muscle mass, strength or athletic skill. Dangerous Misues This misuse of testosterone is indeed very dangerous, and richly deserving of its bad press, because it turns off the user’s own production of testosterone from the testicles and adversely affects the function of adrenal gland. This can in turn lead to infertility, adrenal failure and a life-long struggle to regain health. Black Market and Testosterone Because testosterone is in demand for these illicit purposes, but can only be obtained legally from a licensed physician, a black market has arisen on the internet where weight lifters and athletes can obtain “forms” of testosterone for injection from the eastern bloc countries. I have interviewed body-builders and lifters who get their “testosterone” to improve their bodies or their competitive edge, but they usually have no idea what is really in these injections except what the “seller” says is in them. Nevertheless, in their desire for a better body than God gave them, or for an illegal leg up on the competition, they inject these knock offs, with risks and side-effects too alarming and too numerous to discuss here. CleanTestosterone and the FDA | Testosterone is a Legal Drug In Episode 51 of the BioBalance Healthcast Dr. Maupin and Brett Newcomb discuss how the Food an Drug Administration regulates drugs. The conversation centers around testosterone in treating women's symptoms of menopause. Testosterone was approved for use early 1930s. It is not only legal when administered in synthetic and bioidentical forms by a licensed physician for certain approved conditions, it is an essential hormone that should be replaced in both women and men who suffer from hormone deficiency. In addition to be being legal under appropriate medical conditions, testosterone is regulated, in every form in the United States by the FDA, the Food and Drug Administration. Canada has a similar agency that regulates all drugs. This FDA regulation of synthetic testosterone production by drug companies and pharmacies ensures that: the product is pure the dose on the label is consistent with the drug it is dispensed in a way that leaves a paper trail there are lot numbers and expiration dates on the drug in case there is a recall. The term “approved by the FDA” is different than “regulated by the FDA”. Approval represents yet another level of restriction beyond regulation. When the FDA approves, or “sanctions” drugs, it approves them for a particular purpose or set of purposes, and only that specific purpose or set of purposes. This, however, does not restrict usage beyond those approved parameters. Although bioidentical hormones such as testosterone are not approved for women in pellet form as they are for men, this highly safe and effective treatment is legally, openly, safely and economically available for “off-label use” for women for the treatment of hormone deficiency, just not as widely as it would be were it FDA approved for this purpose. Many physicians, including these conservative ones, often don’t even recognize that some of the medications they routinely prescribe are in fact not approved by the FDA for the disease for which they are using it. The reason is that the use of that drug for that particular purpose, though not approved, is so common that they may even have been trained in medical school to use it for that purpose. BioBalance Healthcast episode 50 is a discussion of breast implants based on a recent New York Times article which quotes Dr. William Maisel from Food and Drug Administration's Center for Devices as saying silicone breast implants are safe, but more study needs to be done. Brett Newcomb comments that in this report, the FDA is sending of "definite maybe" message, and he stresses that careful consideration is still required by women and their physicians before implant surgery is performed. The report mentions potential side effect with breast implant surgery as in any surgery, including infection, wound issues, scarring, pain. Asymmetry is a problem unique to breast implant surgery and can occur based the type of implantation being done. There is a new type of implant on the market that solves the asymmetry problem, as long as both breasts are being replaced, mentioned later in this podcast. Implants are often used by breast cancer patients who have their beasts removed, but are more frequently used for cosmetic reasons. Although Dr. Maupin doesn't do breast implants, she's done breasts exams on thousands of women over the past 25 years, so she knows which of her patients have them. She had found no indication of breast cancer occurring more in women who have had implants verses no implants, or with silicone or saline implants. Sometimes, as a young woman goes through menarche, her breasts grow unevenly. Doctors recommend to waiting until breast development stops, before either performing a reduction on the larger breast, or placing an implant in the smaller one. Contents of breast implants have changed over the years. Silicone implants are bags filled with silicone with a silicone covering. Implants are placed under the skin or under the muscle based on anatomy or preference of the surgeon. Silicone didn't age well, sometimes hardening, leaking or rupturing. Additional surgery would be needed in these cases. There was a huge scare about breast implants a while back that incorrectly tied implants to hormonal problems. Women in their forties were complaining of fibromyalgia, chronic fatigue, rheumatoid arthritis, and other problems, thinking these common symptoms of menopause were the result of their implants. Women were terrified, and had them removed. It's better to have them removed rather than live with the anxiety. Misinformation like this can go viral, so it is up to women to get the correct information. The popularity of saline implants resulted in bad press that silicone implants received. Many women paid to have their silicone implants replace with saline implants. Due to lawsuits, insurance companies were paying women to have this replacement surgery done. Saline implants didn't feel as natural as the silicone implants—feeling more like a bag full of water—and cosmetically didn't look as good. Saline needs to be replaced every ten years. CleanObesity and Hot Flashes in Menopause | Women with a higher BMI may have fewer hot flashes but are in danger of gaining more weight, have a higher incidence of breast cancer and diabetes and more. The subject of episode 48 of the BioBalance Healthcast was inspired by a recently released report by the Endocrine Society that Hot Flashes May be Fewer in Older, Heavier Women. Dr. Maupin and Brett Newcomb discuss what’s in the report and what it really means. Dr. Maupin explains that this report really isn’t news—she has seen this throughout her career as an OB/GYB. Here she explains how women’s hormones work and what happens when the ovaries shut down. In obese women, the body fat produces estrone, or “old lady estrogen” as Dr. Maupin calls it. Estrone production aids in keeping the hot flashes from occurring. Women in menopause who have minimal body fat don’t have the estrone in their system. They have no signals coming from the brain that stop the hot flashes. If your BMI is over 30, you don’t have hot flashes because you’re fat is producing the estrone that keeps them at bay. The bad news is that body fat produces more body fat, creating a cycle of gaining weight. Women with obesity have the greatest risk of breast cancer—thin women have less risk of breast cancer. There is also risk of heart disease, estrone is bad for cholesterol. Diabetes also goes up after menopause. It’s very hard to loose weight after menopause unless you replace estradiol—young women’s estrogen—and testosterone, which makes you leaner. Those two things help make you leaner and stop that process. The study looked at women with a BMI over 30. They proved that women with higher BMI had fewer hot flashes. CleanBioidentical Hormone Replacement Therapy for Symptoms of Menopause | In Dr. Maupin’s appearance in March of last year, she and Dave discussed men and the symptoms of aging they experience due to hormone imbalance and how the symptoms can be treated with bioidentical testosterone pellets. This interview centered around menopause, the symptoms women experience during and as a result of menopause, and why bioidentical hormone therapy with all-natural pellets is the best way to treat many symptoms associated with aging. At the onset of menopause, women sense the physical changes in their body and the change in the way they feel. Often times, their husbands notice the changes as well. As hormone levels drop, women loose their sex drive, they start feeling fatigue, and gain weight, especially in the belly. Mainstream medicine hasn’t yet fully realized the answer, but at BioBalance Health, we have and administer the solution for many of the symptoms of menopause—bioidentical hormone replacement. Within several weeks of pellet insertion sex drive comes back, we get our waistline back; we’re counteracting the first step in again, which is a lack of testosterone. Women lose testosterone and progesterone, then estrogen, estradiol, the young women’s estrogen. Menopause is forever but we don’t have to live with its affects. CleanThe Origins of Our Sexuality | Anthropology, the universality of sex, attraction, instinct, and the formation of family The big picture: Man and Woman has been on the earth over 14,000 years We are the same humans as the original Adam and Eve—in our chemistry, and in the hard wired instinct to have sex, and children. The grand plan as explained by anthropologists, crosses all cultures and exists even today underneath the piles of rules we invent to control our instincts. We were made with a very strong set of instincts, which are hard wired in our brains to insure that human beings would survive into the present. It is located in the brain stem, the “old brain,” and is integral to human nature. The goal of man on earth is to multiply. We are wired to have sex and children as soon as we are mature adults—age 12, and to have as many children as possibly. Humans were not wired monogamously—relationships were meant to last 18 months. That is why we still experience 18 months of extreme neurotransmitters associated with a new relationship. This 18 months gives a couple time to use this attraction to bring 2 people together, have sex, become pregnant, and give birth, accounting for the 9 month gestation. It also keeps the two together for the man to protect the woman and child in infancy, and then move on to produce a child with another woman. Women are hard-wired to be more stable and are the natural caretakers for children. Because of woman’s makeup she can only have one child every two years, but a man can impregnate a new woman at any time. Monogamy is not instinctual. As humans migrated from Africa to all over the world, they took their instincts with them. As human development progressed we covered up our instincts with layers of socialization to cover up our sexuality and inhibit the natural drive in our children to protect families and delay children from having children before they are mature—which gets later and later with each generation. CleanGender Differences in How we Look at Sex and Love | Sex drive for both sexes is defined as interest in sex equals emotional impulses, thinking about sex, fantasies, desire to have sex. This depends on hormones, relationship issues, life stressors, and learned attitudes toward sex and inhibitions. How do men/women define a sexual experience? Sexuality for women is controlled by societal norms much more than men. Women are compelled to suppress their sexuality during the stage of life that correlates with the highest level of hormones…therefore their sexuality is colored by those societal does and don’ts. Most women don’t feel free to be sexual until they are in a committed relationship where they can shed their fears and repression. Men are more free to be sexually open, active and fulfilled, with or without a relationship. After adolescence, men tend to require more of a relationship than just sexuality. Men and women are looking for a satisfying experience, including climax, but women tend to be interested in the pre and post cuddling, holding and connection part of sex, in addition to the climax or climaxes. What is the difference between the sex drive and the sexual experience? Is loving different than sex? If so in what ways? You can love someone you don’t have sex with them. You can have sex with someone who you don’t love. Loving involves intimacy and caring that complete the sexual act, but don’t replace it. Is loving behavior different from sexual behavior. We love our children and take care of them…we stare into their eyes, and feel the joy of melding into their being; Cooing and touching a baby who is nursing at your breast is a loving act but not a sexual one, even though the breast has the sensory nerves that can, in the proper circumstances produce an orgasm, yet in the breast feeing situation stimulates the secretion of prolactin which is a hormone that creates a bond with the baby. What if loving involves sexuality? Does one category subsume the other? One of things I learned from my husband is to inject a little levity when the subject has been deep and emotional for a long-enough period of time. So here goes…In .the movie when Harry Met Sally, Billy Crystal explains to Meg Ryan that men and women cannot under any circumstance be friends, because he explains that if a man is a close friend (which I assume means friends who love each other) with a woman, it inevitably leads to sex. Without that outcome , he believes that the couple weren’t really friends if this outcome is not achieved. The movie goes on to prove that Harry and Sally eventually prove that this belief comes true, even if for a limited period of time. My read on that is that men(at least Harry) believe that loving a female friend is the premiere to sex, while women don’t generally have that belief, but they often subsume their friendship to sex. The use of generic labels such as “hormones” are misleading because they are non specific, and do not make use of distinctions among specific hormone groups that have clinical significance . Historically in medical research women have been minorities as research groups. Much of what was researched and the conclusions from the research was generalized from research done on men. One of the reasons for this was a concern about doing research on women who might be pregnant. Only in the last decade or so has serious broad spectrum research focusing on women been at the forefront of medical science. Can you speak to the reasons for these distinctions and the changes in the way science is beginning to look at medical research data on women. There was a period of time when hormone replacement therapy was the gold standard for the treatment of menopausal symptoms and the treatment of aging issues in women. There were 25000 studies documenting the efficacy of estrogen replacement for women. Then the WHI study came out in 2002. How has this changed thinking with regard to treating women? There is a story in the press today about hormone therapy increasing the risk of breast cancer. In a logic class this would be an example of what is called an undistributed major term. What does that mean and how does it apply to the article today and to other articles that appear in the mass media? Some people are afraid to consider hormone replacement therapy because they have heard “things” about it: they have heard that it causes cancer they do not make distinctions between types of hormones they do not know or make distinctions among types of or options for hormone replacements there are several types of hormone replacements delivery systems. What are the different types of hormone replacements or delivery systems that are available? Why are bioidenticals better? Why do pellets work better for what you do than other types of applications? Example of flirting in a marriage was obvious in the remake of the Untouchable when Elliot Ness (played by Kevin Costner) and his wife Catherine are flirting on the front staircase of their home after a very stressful day. Catherine says, to her husband, “Do you want to come upstairs and brush my hair?” which elicits the invited interaction. Why is it sometimes problematic in relationships Flirting isn’t straightforward..it is very indirect Not clear communication to one partner When it is used to cause jealousy in a partner The flirting is consciously ignored Where is it directed? Is it threatening subjectively or objectively Is it ever a good thing? Dr. Maupin: Well, Brett. We're talking today about sexual dysfunction and some of the things that I see in the office that I treat with BioBalance pellets and testosterone. You come from more of the sociologic, psychological point of view of how you counsel your patients. Mr. Newcomb: And, even anthropological. Because so much of it is cultural. The definition of beauty is a culturally derived concept. There are cultures in the South Pacific where food is in short supply, and it is a status and beauty marker for the dominant male to have multiple wives, all of whom are 300 pounds or more. So, a woman that large in that community is a walking statement about virility and sexuality and attraction. But in other communities, a woman that size is not. Dr. Maupin: But, there are some anthropologic qualities that go across all cultures like healthy looking, good skin. Also, having a waistline, whether you're heavy or not, indicates fertility in females. Generally, height in all cultures for males shows a dominant male. So, there are some that cross cultures.
