"ADHD" is the shorter term for "Attention Deficit/Hyperactivity Disorder". ADHD is diagnosed at a clinical setting by mental health professionals, based on established diagnostic criteria, through a face-to-face evaluation. The following "test" is a group of questions inquiring about ADHD symptoms. The result is given as "YES", "MAYBE" or "NO" depending on whether there are ENOUGH, SOME or NO positive clinical symptoms, respectively.
You can screen yourself or your child, and you are able to email the POSITIVE symptoms to yourself, to easily share them with your counselor, primary care physician, etc.

This simple application will give you an opportunity to accurately screen your child, adolescent and adult patients for ADHD. You can email the positive symptoms to yourself to print and put into the patient's chart and/or to copy and paste the positive symptoms into your letter when referring your patient to a mental health professional.
