This warfarin initial dose estimator is presented by The Institute for Pharmacogenomics and Individualized Therapy (IPIT) at the University of North Carolina at Chapel Hill based on the International Warfarin Pharmacogenetics Consortium warfarin dosing algorithm.
Height and weight can be entered in metric or English units.
