RequestIndy is an exciting tool from the City of Indianapolis’ Mayor’s Action Center that allows our citizens to request non-emergency City services and report issues such as potholes, missed trash collection, and stray animals. Currently, RequestIndy can be used to enter more than 30 of the most commonly requested services. RequestIndy is fully-integrated with the City’s enterprise-wide service systems for each City department so requests are routed directly to the folks who can resolve the issue.
RequestIndy is available 24 hours a day, 7 days a week to allow citizens to make these requests at their convenience.
RequestIndy may also be used to check the status of previously submitted requests.

Great, when it works
I've used this for awhile. It's useful when it works; now it just shows the splash screen then closes so I can't use it. Tried reinstalling to no avail. Also, the Comments section doesn't show the cursor when you type, so if you need to go back and change something, you just have to guess where you're typing. And I agree that getting comments back from the city would make this app tremendously useful: as it is, when I file a request, I simply have to hope they got it and acted on it, as I typically don't get emails back with a response.
Good app but needs closure comments
This is a great app for reporting issues, but there needs to be a way to review the closure comments. I reported two separate issues for red light signals out, and both issues were closed. I had to call in to find out why they were closed since the lights were not fixed, and I was told that they had been transferred to IN-DOT. I would not have needed to call to check on the issues if the closure comment had been available on the app.
Connection to city
Before I had this app, I had never reported a problem to the city even though I often saw reportable issues like potholes or stray animals. I never knew the number to the Mayor's Action Center. Now I can quickly & easily report these issues. Plus I can send a photo with the report & even know the status of my report. Great application! A broader audience is being reached.
