A rogue FBI agent takes Dr Ellen Sanders hostage after she is chosen to operate on the president, threatening to kill her and her family unless she assassinates him during the surgery.

When Ellen disobeys Duncan's orders and stalls the attempt to assassinate the President during surgery, he threatens to take the life of a member of her family. Ellen begins to realise that there is no one she can trust.

When President Kincaid decides to use the services of another surgeon, Duncan threatens Ellen that there will be terrible consequences if she can't convince him to keep her on as his doctor.

Ellen and Brian come up with an escape plan, but can Brian manage to distract Duncan and his team for long enough to allow Ellen and the children to flee to freedom?

Ellen has to make an agonising choice between saving her husband and protecting her children, and she finds herself forced to perform emergency surgery in her own home.

When Ellen's sister, Lauren, turns up unexpectedly at the Sanders's house intending to stay, Duncan forces Ellen to find a way to get rid of her sibling without arousing suspicion.
