Isabel provides a short list of possibilities from which you may build your differential or use as a trigger to obtain further information from your patient to assist in your differential workup.
It is not designed to provide a diagnosis but rather provide you with a list of likely diagnoses to consider when you have doubt. A good example would be a patient in ED with hand edema and hypotension who you are treating for sepsis and is not responding. The Isabel system might remind you to also consider anaphylaxis among other suggestions.

I have only used Isabel a handful of times on outpatients, so my first impressions may be inconsiderate of its full potential. Isabel would be a bit more helpful if it could rank possible diagnosis’s in order of likelihood. It does however highlight the more critical diagnosis’s. It’s not helpful for looking up a differential of possible causes for a single isolated finding such as elevated bilirubin, as you will have to wade thru many irrelevant diagnosis’s to find the more pertinent one’s. I suspect it will be most helpful for inpatient and ER cases where the diagnosis is not immediately apparent.
I did not buy and do not use this app.
I read about this app in a journal, they said it was excellent, but cumbersome. I went to download it and was struck by several things: one, this hasn't been updated since January 2014. Two, there are very few reviews. Three, I would have liked a short trial to be able to see how it works for me, definitely don't want to pay for that opportunity.
The product is so-so for Neurology DDx. A reasonable list may be generated although most are quite incomplete. Ok for a start. Not a place to find which spinocerebellar atrophy or paraneoplastic syndrome is likely.
