Arrival times can be accessed using WiFi, 3g, and Edge. Route and stop information is saved on your device so you can view them offline. If MBTA has a major route change, simply press one button and Pocket MBTA will download all of the updates.
IMPORTANT NOTE: Vehicle locations are not supported for every line. This will NOT affect the accuracy of displayed arrival times.

One of the Top Choices for NextBus
In the interest of Science, I've installed and tried every NextBus-enabled app available. They're all getting the same data, but this one has a nice clean UI and good basic features.
The user interface is clearly designed to make frequent operations of a typical commuter quickly available. Some other apps waste screen real estate with top-level buttons devoted to infrequent operations like deciding which city you're in. This app focuses on two use-cases: a) when is a bus coming to one of my "favorite" bus stops?, and b) what's around me now and where can I go, when? And it does those things very well. And you can also look up stops by route if need be.
Update: a few bugs I had mentioned here previously are fixed. Nice! The developer is very responsive.
Good but inaccurate
I take the 57 every day and a few months ago, in a route update, the app changed the route to say that the 57 goes all the way to Haymarket instead of stopping at Kenmore. The 57 DOES NOT go all the way to Haymarket! Kenmore is the last stop on this route, and I have no idea why they changed the route. This inaccuracy also causes the buses' locations to not show up on the map, which is my favorite feature of the app. I now manually update the routes about once a week in the hopes that they fix this error. Until they do, this app is stuck at three stars with me, despite otherwise being useful. Please fix!
It is NOT the whole MBTA
This app is just for buses. Commuter Rail, subway and boats are NOT included. So DON'T believe the title -- you aren't getting the MBTA. The app description is unclear -- it talks about buses, but doesn't exclude other modes. But the title definitely implies all modes, so it's very misleading.
Also, even for buses it is just so-so. There is no system map. If you are interested in going to a place and wonder what buses go near there you are out of luck with this app.
I gave it two stars because for the situation where you know your bus route and want to look at schedules, it seems to be pretty accurate.
