StopRSI now works in the background on iOS 4. It's simple:
1) Open app
2) Start work session
3) Close app
Notifications will be configured for the beginning and end of your micro pauses and work breaks for the next 8 hours. Want to stop the notifications? Just open the app and close the work session (tap the X in the top-left corner) and then close the app. All the notifications will be removed.
WARNING: Always use good judgement and consult your doctor before beginning any program of exercise.
NOTE: Vibration alerts are available only on the iPhone. 1st generation iPod touch users will need to have headphones or speakers plugged in to hear audio alerts.
BACKGROUND NOTE: Ringer must be turned up to hear notification tone. In either case a badge will appear on the home screen icon indicating how many seconds your break will last.

I love having a regular reminder for stretching but not at this rate of battery use.
When I run this app my iPhone4 battery drains down to 40 % in a matter of 3 to 4 hours and feels hot to touch even when all other apps are turned off.
Attempted to use in background mode, but I don't consistently receive alerts. This is on an iPhone 4 running latest iOS 5.
with preferences set to sound off vibration on it still makes loud beeps on ios5. unusable around coworkers.
