Clearly lists all changes I can make to my habits to lower disease risk. It'd be really neat if it showed probabilities in addition to "risk vs others in cohort". Two levels of exercise would also be neat (basic activity levels and more athletic activity levels)

Good product but,after taking the survey I find the app no longer needed. It lost its usefulness. Now if the developer created an actual tracking program where you can add activities and foods eaten and it would calculate your risk factor; then this app would retain it's usefulness.
Good idea, bad information
A very good idea. Sadly, it follows the US Government Healy guidelines for eating. These are horribly wrong and based on flawed studies. See, for example, "Low Carb Living" by Bowden, "Why We Get" Fat by Taubes, "The Art And Science of Low Carb Living" by Phinney and Volek for extensive refutation of government guidelines.
So, if you subscribe the the conventional wisdom, this is for you. If you are low carb or paleo, look elsewhere.
